I.        ASSIGNED DUTIES & TASKS:

 

Position overview:

This position is responsible to function as part of a disease investigation and intervention team; plan and participate in programs related to the control of various communicable diseases; to locate disease sources, identify at-risk groups to prevent, treat and thereby reduce the incidence and risk to citizens; to provide education to patients diagnosed positive for a communicable disease, and to provide information on disease etiology and the prevention of communicable diseases.

 

All duties below are essential functions unless otherwise indicated. Duties of the position are not all inclusive and subject to change.

 

PUBLIC HEALTH RESPONSIBILITIES:

 

  • The Disease Intervention Specialist is responsible for interviewing cases, contacts and others in the community concerning personal behaviors and activities which contribute to disease transmission; to make collateral contacts to obtain pertinent information concerning other individuals who may have been exposed or infected; to ensure compliance with established State and federal laws and regulations related to disease control and prevention.
  • Act as patient advocate; educate clients regarding medical and social services available and intervene on their behalf for access to services or resolution of conflict.
  • Conduct case investigations on infected patients; documents case histories and possible infection sources.
  • Conduct timely vaccine clinics for community members and local businesses during appropriate time of year.
  • Assist with Public Health Emergency Preparedness grant immunization requirements
  • Answer disease related questions.
  • Keep current with local, national and international disease trends and outbreaks.
  • Produce contact reports and tracing records.

II.      KNOWLEDGE AND SKILLS

KNOWLEDGE:

·         This position requires a strict understanding of patient confidentiality, including the ability to conduct interviews without violating confidentiality.

·         Understanding of medical terms and principles of exposure infection, infectious  period, potentially infectious interactions, symptoms of disease; pre-symptomatic and asymptomatic infection.

·         Assist with obtaining current information pertaining to the symptoms, modes of transmission, control, treatment, and effects of the communicable/chronic disease assigned.

·         Understanding of CDC protocols for investigation and intervention with patients who have positive results for a communicable disease.

·         Knowledge of Modern practices and principles of medical disease counseling.

·         Principles and procedures of record keeping; medical documentation; business writing.

·         Universal (Standard) precautions and infection control practices.

 

SKILLS:

·         This position requires excellent and sensitive interpersonal, cultural sensitivity, and interviewing skills so that trust can be built and maintained with patients and contacts.

·         Understanding of when to refer individuals or situations to medical, social or supervisory resources.

·         Maintain active surveillance of communicable diseases through Park County.

·         Assist with enforcement of quarantine as needed, with guidance provided by Health Officer and Board of Health.

·         Work in conjunction with Park County DES on countywide emergency protocols.  

·         Ability to interact with the public calmly and effectively.

·         Strong listening, verbal and written communication skills.

·         Ability to work with a high level of detail in a fast paced environment.

 

Education Education and Experience

Must have a degree (minimum Associates) in Nursing. Experience working as a nurse with 3-5 years is preferred. Experience working with the general public and people of varying backgrounds is needed. Experience working in the public health field would be terrific!

Certification, Licenses, etc.

Montana State Board of Nursing License – Active; FEMA IS-700, ICS-100 online courses within 6 months of employment and ICS-s00, ICS-300 courses within one year of employment as mandated by the state.

Valid Driver’s license. 

III.        ACCOUNTABILITY

This position makes complex decisions regarding the public health team goals and objectives. I.        ASSIGNED DUTIES & TASKS:

 

Position overview:

This position is responsible for highly complex administrative work involving the planning, organization, direction, supervision and coordination of the operations of the Park County Health Department, including its Environmental Health and Human Services. The nature of the work performed requires that an employee in this class establish and maintain close cooperative working relationships with the Board of Health, the health officer, health department team, public officials, collaborating organizations and agencies, compliance agencies, other county employees and the general public.

 

All duties below are essential functions unless otherwise indicated. Duties of the position are not all inclusive and subject to change.

PUBLIC HEALTH DUTIES:

 

  1. To develop goals, plans, initiatives and measurements for the identification and evaluation of public health needs and services and provide continuous quality improvement.

 

  1. To successfully confer with community leaders and other health and environmental professionals to discuss, identify and assess public health needs and problems.

 

  1. To accurately identify the costs, timing and other key issues involved in the expansion and improvement of public health services.

 

  1. To develop recommendations for expanding and enhancing public health service and help identify and secure funding for services.

 

  1. To plan and carry out public education programs and activities.
  2. To develop, monitor and administer department budget and fiscal contracts for all pillars of the Health Department including Health Promotion/Administration, Clinical Nursing and Environmental Health

 

  1. To collect and analyze data and information pertaining to department activities and finances and prepare and submit reports as requested by the County Commission and Board of Health.

 

  1. To meet and confer with citizens, community groups, members of the medical and public health community and others to discuss department policies, programs and services. To be an effective public relations presence in the community.

 

  1. To ensure community compliance with applicable state and local health laws and regulations; monitor effectiveness and make recommendations for changes as justified.

 

  1. To build and support the operation and services of all public health nursing and wellness department programs.

 

  1. In consultation with the Director of Grants & Special Projects (DGPS), make decisions on which grants will be pursued to the Health Department and on developing and maintaining an annual calendar of planned and ad-hoc grant development activities. Also work with DGSP to ensure all aspects of the Health Department’s grant approval process are followed, including review and grant applications; obtaining appropriate signatures; and, incorporating maximum possible indirect cost recovery.

 

  1. To oversee the operation and services for environmental health in nuisance control, water and sewage, rodent and insect control, air pollution, water quality, lead poisoning prevention, radiation and housing.

 

  1. To review citizen complaints associated with Health department programs and provide responsible systematic and successful approaches. To assist in coordination of issue analysis or other concerns presented by the Board of Health or County Commissioners.

 

  1. To plan, prepare, attend and serve as secretary to the Board of Health or designate a health department team member to serve the Board in that capacity.

 

 

  1. To attend a wide variety of county, community and statewide meetings, conferences and workshops as requested and authorized.

 

  1. In the event of an emergency this position most generally will be placed under the command of or delegated to by the Incident Command. The Health Department Director will be asked to perform those duties that are assigned by the Incident Commander.

 

ADMINISTRATIVE RESPONSIBILITIES:

 

  1. To direct, supervise, motivate, counsel, train, prioritize and evaluate the work and performance of  theHealth Department team.  

 

  1. To recommend new hires and discipline and termination actions for department team.

 

  1. Work with DGSP to develop and maintain systems and procedures for grant administration activities to include (but not necessarily limited to):

 

  • To assist in project management related to grant application project goals and activities;
  • To assist in the administration of grant funds received to include preparation of requests for qualifications and/or proposals and project implementation;
  • To prepare quarterly narrative and programmatic reporting and quarterly reimbursement requests; and
  • To prepare grant closeout documentation which may include final narrative and programmatic reporting and final reimbursement requests.

 

 

OTHER DUTIES AS ASSIGNED:

Performs other duties as assigned by the County Commission. This includes special projects and a variety of other functions as determined.

II.      KNOWLEDGE AND SKILLS

 

Knowledge of public health and/or human services program principals; knowledge of budget development and management; knowledge of grants administration; knowledge of long and short-range program development; knowledge of local government policy and decision-making process; knowledge of project management principles and practices; knowledge of team leadership motivation; ability to establish and maintain effective working relationships. Ability to communicate and negotiate effectively among diverse interests and groups. Comfortable with public speaking. Ability to work after hours and on-call, longer shifts, weekends, etc. to meet the operational needs of the department. Computer skills such as; email, word processing, excel, power point – to establish databases, spreadsheets, presentations and to complete research of health related issues.

 

Education and Experience

Bachelor’s Degree in Public Health, Business or Health related field. Master’s Degree in Public Health, Public Administration, or closely related field is preferred. Minimum of three years of professional level experience in leading community initiatives and/or facilitating teams focused on public health issues, strongly desired.  Experience in performance management, public health accreditation, quality improvement, strategic planning, community health improvement planning, communicable disease follow-up, health hazard and food/water-borne outbreak investigation is preferred. Effective public speaking skills, highly proficient personal computer software applications skills, willingness and ability to attend evening meetings, travel out of town and attend workshops, conference and meetings during work and non-work hours.

 

Certifications, Licenses, etc.

Valid Driver’s license. Must obtain a Montana Driver’s license within 60 days.

Complete emergency preparedness training and provide certification of completion within 120 days of hire.
